What is IPL?

Intense Pulsed Light (abbreviated IPL) describes the use of intense pulses of non-coherent light distributed over a range of wavelengths; from 515nm to 1200nm.  The technology utilizes specific wavelength ranges to target various chromophores in the skin. This enables effective treatment of a broad spectrum of conditions, including vascular and pigmented lesions, skin photoaging treatments and hair removal.

How IPL works?

A broad spectrum of light is transmitted through a transparent handpiece tip which is gently placed over the skin. Cut-off filters in the handpiece change the range of wavelengths, optimizing it for different applications and skin types. The light penetrates the tissue and is absorbed by either the relevant chromophores and damages them (blood chromophores when treating vascular lesions and melanin chromophores when treating pigmented lesions).  The body’s natural processes then clear the lesion debris, giving the skin a more even and youthful appearance.

About Pigmentation

Pigmented lesions are hypermelanocytic areas typically induced by sun exposure, the natural aging process or congenital factors. Pigmented lesions vary widely in size, shape and depth. They can be either raised or flat and are usually a cosmetic rather than a medical concern. Intense Pulsed Light (IPL) which targets the melanin is effective in treating benign lesions and clearing them.

About Rosacea Treatment

Rosacea is a chronic skin condition which expresses itself in redness on the face and sometimes on the neck and chest. The exact cause of Rosacea is yet unknown and many who are affected by it are unaware of their condition. The IPL procedure removes facial superficial vessels and as a result dramatically decreases or completely eliminates facial redness. The vascular components of Rosacea can vary from flushing to telangiectasia and can have different densities and depths. With a choice of multiple vascular cut-off filters and unique pulse sequencing, Lumenis’ IPL with OPT can coagulate each sub-type of capillaries, reducing the redness and simultaneously triggering neocollagenesis which limits the reccurrence of the condition.

What is a long pulsed Nd:YAG laser?

An Nd:YAG laser is a solid state laser capable of producing a near-infrared wavelength that penetrates deep into the skin and is readily absorbed by hemoglobin and melanin chromophores. The lasing medium of Nd:YAG (Neodymium-doped Yttrium Aluminum Garnet) is a man-made crystal (solid state) that is pumped by a high intensity lamp and placed into a resonator (a cavity capable of amplifying the power of the laser). By creating a variable long pulse duration and an appropriate spot size, it is possible to significantly heat deep skin tissues, such as large blood vessels and vascular lesions.

How does a long pulsed Nd:YAG laser work?

Nd:YAG laser energy is selectively absorbed by the deeper levels of the dermis and allows for treatment of deeper vascular lesions such as telangiectasias, hemangiomas and leg veins.   The laser energy is delivered using long pulses which are converted into heat in the tissue.  The heat impacts the vasculature of the lesions. In addition, the Nd:YAG Laser can treat at a more superficial level; by heating  the subcutaneous skin (in a non-ablative manner) it stimulates neocollagenesis which improves the appearance of facial wrinkles.

Lumenis’ long pulsed Nd:YAG laser is available as part of the M22 multi-application platform.

By utilizing the same power supply for both the IPL and the Nd:YAG laser, Lumenis is able to provide more treatment versatility in a single platform.

Multiple Sequential Pulsing (MSP) is available in both the Nd:YAG and the IPL modules on the M22 platform.  MSP allows for cooling between a sequence of pulses which protects the epidermis and allows for safe use of higher fluence.  The MSP offers safe treatment of all skin types, including dark skin, and significantly reduces side-effects in treatment of vascular lesions.

About Tattoo Removal

Tattoo dyes consist of pigmented solid particles. In the same way as other pigmented lesions, these pigmented particles can be targeted by a dedicated laser and eliminated.

Different tattoos might require different types of laser in order to remove them, depending on the type of ink, the color, and the depth of the tattoo.

Q-Switch lasers release high-powered pulses of extremely short duration. The intense and rapid heating induced by light absorption leads to a photo-thermal effect. As the chromophore is heated, it undergoes sudden thermal expansion generating shock waves audible as a popping sound with each laser pulse. This photo-acoustic effect generates forces that allow fragmenting tattoo pigment particles. The pigment treated this way gradually becomes shallower over time until it fades away. The peripheral tissues remain unaltered making the treatment selective and safe.

Lumenis M22 IPL with OPT™

The Lumenis IPL with OPT™ (Optimal Pulse Technology) is designed to treat vascular lesions up to 1.5 mm deep and up to 1 mm in diameter.

The Multi-Spot Nd:YAG with Multiple Sequential Pulsing (MSP) effectively treats vascular lesions up to 4.5 mm deep and with a diameter of up to 4 mm,  such as leg veins.  The Multi-Spot Nd:YAG treats all skin types safely and effectively.

About Laser Hair Removal

Lasers remove hair through a process of selective photothermolysis in which energy in the form of optical wavelengths is delivered into the hair shaft and pigmented follicular epithelium. The light is primarily absorbed by the endogenous chromophore melanin and results in a rapid rise in temperature and subsequent heat transfer to adjacent tissue, causing local thermal necrosis of the follicles’ regenerative structures.

To achieve permanent hair removal, the laser system must be able to effectively treat the different hair types and colors as well as the various skin types while providing maximum skin protection.   The laser’s wavelength, pulse duration and spot size determine the effectiveness of the treatment (on all skin types) and also influence the depth of penetration and treatment speed.

About Skin Toning

Skin toning refers to the use of Q-Switched lasers for non ablative skin rejuvenation.

This term is more common in Asian countries.

In skin toning, multiple passes of low-dose Q-Switched Nd:YAG are delivered through a large spot size to induce sublethal injury to melanosomes causing fragmentation and rupture of melanin granules into the cytoplasm. This effect is highly selective for melanosomes and leads to overall skin brightness. Moreover, the subthreshold injury to the surrounding dermis stimulates the formation of collagen that results in tighter skin.

The Lumenis Q-Switched Nd:YAG was developed to enable skin toning, by treatment of pigmented lesions, using a large  and non-disposable tip of 6mm. Using a top-hat beam profile ensures control of homogeneous energy delivery and maximum safety without hot-spots nor any risk of side effects or complication for the patient.
